Authorization to Release Information Form

This article provides instructions to access the Release of Information form for students.

Instructions

1. Go to the Access to Student Information page.
2. Scroll down to the bottom of the page and select the Release of Information Form link.

NOTE: This form does NOT give the parents authorization to reset the student's password.
