Authorization to Release Information Form

Overview

This article provides instructions to access the Release of Information form for students.

 

Instructions

  1. Go to the JCCC home page.
  2. Select Menu.
  3. Select  Student Resources.
  4. Select Student Records from the left panel.
  5. Select Access to Student Information from the expanded left panel.
  6. Scroll down to the bottom of the page and select the Release of Information Form link.

NOTE: This form does NOT give the parents authorization to reset the student's password.
